Pre-Advising Questionnaire:

Students who wish to meet with an OPGSA advisor for the first time should first call 979-847-8938 to make an appointment, and then they must complete one of the following questionnaires before arriving for their advising appointment.


1. Download the appropriate questionnaire above

2. Complete the PDF and print it out OR print it out and complete it by hand

3. Bring completed questionnaire to your first scheduled advising appointment.

First-time students who attempt to check in for a scheduled appointment without a completed questionnaire will be asked to complete one on site, thereby shortening the length of their appointment time.
